CASE REPORT Secondary Endolymphatic Hydrops Following Sudden Deafness Detected by MRI after Intratympanic Administration of Gadolinium

چکیده
the hydrops by image analyses. A 64 years-old female who showed partial recovery of hearing disturbance two months after the onset of sudden deafness underwent inner ear MRI in combination with intratympanic injection of gadolinium. Two-dimensional fluid-attenuated inversion recovery (2D-FLAIR) sequences taken by 3 tesla MR unit showed endolymphatic hydrops in the cochlea. Electrocochleogram could not be performed due to high tone hearing loss. At present, inner ear MRI in combination with intratympanic injection of gadolinium has been mainly used for Meniereʼs patients to detect endolymphatic hydrops. However, this tool cannot distinguish the secondary hydrops as seen in our case from Meniereʼs disease and thus should be used with careful attention if used as a routine test for inner ear diseases.

Imaging of endolymphatic and perilymphatic fluid at 3T after intratympanic administration of gadolinium-diethylene-triamine pentaacetic acid.
By optimizing the inversion time of a 3D inversion-recovery turbo spin-echo sequence at 3T, we obtained separate images of endolymphatic and perilymphatic space 24 hours after intratympanic administration of gadolinium contrast material. In patients with Ménière disease, endolymphatic hydrops were detected not only in the cochlea but also in the vestibule. Fusion of the 2 types of images visual...
متن کاملEndolymphatic Hydrops Detected by 3-Dimensional Fluid-Attenuated Inversion Recovery MRI following Intratympanic Injection of Gadolinium in the Asymptomatic Contralateral Ears of Patients with Unilateral Ménière’s Disease
BACKGROUND The aim of this study was to identify the incidence of endolymphatic hydrops using 3-dimensional fluid-attenuated inversion recovery (3D-FLAIR) magnetic resonance imaging (MRI) in the contralateral ear in patients with unilateral Ménière's disease (MD). MATERIAL AND METHODS This was a prospective study. 3D-FLAIR MRI was performed with a 3 Tesla (3 T) unit 24 h after the intratympan...
متن کاملComparison of the diagnostic value of 3 T MRI after intratympanic injection of GBCA, electrocochleography, and the glycerol test in patients with Meniere's disease
CONCLUSION 3 T MRI after intratympanic injection of gadolinium-based contrast agent (GBCA) is more useful for the diagnosis of endolymphatic hydrops compared with the glycerol test and electrocochleography (ECoG). OBJECTIVE To investigate the relationship between 3 T MRI after intratympanic injection of GBCA, the glycerol test, and ECoG in patients with Meniere's disease (MD). METHODS A tot...
متن کاملVasopressin induces endolymphatic hydrops in mouse inner ear, as evaluated with repeated 9.4 T MRI.
From histopathological specimens, endolymphatic hydrops has been demonstrated in association with inner ear disorders. Recent studies have observed findings suggestive of hydrops using MRI in humans. Previous studies suggest that vasopressin may play a critical role in endolymph homeostasis and may be involved in the development of Ménière's disease. In this study we evaluate the effect of vaso...
متن کاملSemi-Quantitative vs. Volumetric Determination of Endolymphatic Space in Menière’s Disease Using Endolymphatic Hydrops 3T-HR-MRI after Intravenous Gadolinium Injection
Magnetic resonance imaging enhances the clinical diagnosis of Menière's disease. This is accomplished by in vivo detection of endolymphatic hydrops, which are graded using different semi-quantitative grading systems. We evaluated an established, semi-quantitative endolymphatic hydrops score and with a quantitative method for volumetric assessment of the endolymphatic size. 11 patients with Meni...
